As we transition from a fee-for-service model to value-based care, we seek a dedicated and experienced Vice President of Clinical Education to lead our educational initiatives and support our team through this significant change.

The Opportunity

As a Provider at Pine Park Health, you will be at the forefront of delivering personalized, in-person primary care to a high-acuity geriatric population residing in senior living communities. You will serve as the cornerstone in a comprehensive healthcare model that focuses on clinical excellence, meaningful community engagement, and proactive patient education. You'll be responsible for a spectrum of care—from preventive and routine care to urgent clinical interventions—all while fostering strong relationships within the community. Leveraging your skills in diagnostics, treatment planning, and health promotion, you will work closely with our interdisciplinary team to achieve superior patient outcomes and elevate the standard of care in geriatric medicine. As Director of Clinical Quality, you will collaborate with clinical teams to meet quality standards and regulatory requirements. Responsible for quarterly OKRs, you will work on improving workflow efficiency, documentation quality, and value-based performance. You will develop and implement quality improvement initiatives, conduct audits, analyze data to track effectiveness, and stay updated on industry trends and best practices.

Key Evaluation Metrics: Success will be measured in the following focus areas:

  1. Visit Note Closure Rate: All chart notes will be closed within two business days.
  2. Net Promoter Scores (NPS)
  3. Number of “Visit” Frequency, i.e., patient visit notes, phone notes, and community check-ins
  4. Review of PineCone CX recommendations
  5. Ongoing improvement and/or maintenance of market metrics based on quarterly OKRs

Performance focus areas and goals may evolve to align with changing business objectives.

What You Will Do

  1. Clinical Patient-Facing Duties:
  • Conduct comprehensive medical assessments, including physical examinations and medical history reviews, for residents in the assisted living facility.
  • Make accurate diagnoses based on assessments and collaborate with the healthcare team to develop appropriate care plans.
  • Implement and coordinate healthcare interventions, including medication management, wound care, chronic disease management, and health promotion activities.
  • Provide education to residents, families, and staff on disease prevention, self-care techniques, and the management of chronic conditions.
  • Monitor and manage chronic illnesses, adjusting treatment plans and medications as necessary, in collaboration with physicians and specialists.
  • Engage with patients, families, and staff to create an environment of trust, ensuring open lines of communication to discuss care plans, updates, and changes.
  • Maintains a patient panel of approximately 130-220; caseload will vary based on patient acuity and geographical distribution.
  1. Clinical Excellence and Quality Improvement:
  • Collaborate with clinical teams to ensure adherence to quality standards and regulatory requirements.
  • Be accountable for quarterly organizational OKRs (Objectives and Key Results) as determined by leadership, such as improving workflow efficiency, quality of documentation, and value-based performance.
  • Develop and implement quality improvement initiatives to optimize clinical processes and workflows.
  • Conduct regular audits and assessments to identify areas for improvement and ensure compliance.
  • Provide clinical insights and support that complement the work of product and data analytics teams, focusing on areas where clinical knowledge is essential.
  • Stay updated on industry trends, regulations, and best practices in clinical quality management.
  • Act as an advocate for patient-centric care within the clinical leadership team, voicing ideas, concerns, and innovative solutions.
  1. Community Collaboration:
  • Clinical Collaboration meeting with new communities.
  • Conduct rounds at new or established communities with the Enrollment Director.
  • Hosting informational sessions such as the 4Ms talk and managing an informational table with the Enrollment Team.
  • Lead community education initiatives to improve the overall health literacy of the senior living communities we serve.
  • Establish yourself as a visible healthcare leader within the community, attending key meetings and events to build trust and strengthen our partnerships.
  • Engage in projects such as in-service for community staff, drop-ins for vitamin and supplement review with providers, health fairs for blood pressure checks, and flu clinics.
  1. Clinical Leadership:
  • Provided day-to-day management, overseeing panel assignments and staffing and ensuring adaptability to organizational growth of clinical teams in identified markets based on evaluation of market performance as determined by CMO.
  • Expand role within executive leadership team as determined CMO.
  • Conduct regular semi-monthly meetings with assigned Providers to review performance, address concerns, and offer guidance.
  • Other duties as assigned.

What We Are Looking For

  • Proven experience working as a Nurse Practitioner, preferably in a geriatric or assisted living setting.
  • Comprehensive knowledge of geriatric healthcare, chronic disease management, and preventive care.
  • Strong clinical assessment and diagnostic skills, with the ability to formulate and implement effective care plans.
  • Proficiency in medication management and the ability to administer treatments and medications.
  • Excellent communication and interpersonal skills, with the ability to interact compassionately with residents, families, and staff.
  • Ability to work independently and as part of a team, demonstrating flexibility and adaptability in a dynamic healthcare environment.
  • Ability to perform multiple tasks and work in a fast-paced environment.
  • Proficient computer skills, including electronic health record (EHR) systems and documentation.
  • Experience collaborating with growth, product, and clinical teams to improve clinician experience and metrics.
  • Experience 5 years as an advanced practice provider.
  • Minimum of 3-5 years of direct management experience in a clinical or healthcare setting, with a proven track record of successful team leadership and operational oversight.

Education and Experience

  • Board-certified nurse practitioners preferably have 3+ years of geriatrics experience and are licensed or eligible for licensing in AZ, CA, and NV. Certification as a Nurse Practitioner by a recognized certifying body (e.g., AANP or ANCC).
  • Master's degree in Nursing, with a specialization in Family or Adult-Gerontology.
  • Valid DEA Certificate in good standing for Schedule 2-5 prescriptions.
  • Not under current exclusion or sanction by any state or federal health care program, including Medicare or Medicaid, and is not identified as an “excluded person” by the Office of Inspector General of the Department of Health and Human Services or the General Service Administration (GSA), or reprimanded or sanctioned by Medicare. Must maintain Medicare active status.
